I have used this tool in the past to capture specific activities and actions taken. This tool can be set up by processes in your organization, or specific knowledge areas, and I have also seen it deployed in support of hardware quality learning over time.

The tool supports distributed teams and in essence is a written log of things learned (or it can be if you take the appropriate actions). We don't need to burn our hand on the same stove over and over again, record it, get you people to know it and go to this area for information.
