I am re-reading some process management material, and had a flashback to my employment over the years. I have heard many in the organization complain about the lack of capability of the processes of the company. The problem was, the process was infrequently followed. Upon in review of this, I asked the question, "How would you know the capability of the process when many times it is circumvented and this is not accounted for in the measurement (if there is even a measurement)?"
